New Patient Pre-Registration
Welcome. A few minutes now means less paperwork later.
If this is your first visit with Dr. Pandey, here's how to get set up so your appointment starts on time and the front desk isn't waiting on forms.
Step 1. Book your first visit
Reserve a time online through Zocdoc, or call the office. In-person and video visits are both available.
- Book online: use the Book Online button at the top of this page.
- Prefer to call? The office is at (212) 964-0971, Monday to Friday, 10am to 6pm.
Step 2. Pre-register online
Pre-registering fills in your details ahead of time, so check-in on the day takes a couple of minutes instead of fifteen. Open the portal sign-in page (the Sign In or Book Online link), then look for these two options:
- Under New to Practice?, choose Pre-register to add your information before your first visit.
- Under Don't have a Portal account?, choose Create a new account to set up your username and password so you can message the office and see results later.
You'll be asked for your name and date of birth, contact details, insurance, current medications, allergies, and the reason for your visit. Everything is private and encrypted.
What to bring to your first visit
Photo ID
A driver's license or any government-issued ID.
Insurance card
For primary care visits, so we can verify coverage.
Your medications
A list, or the bottles, including doses and anything over the counter.
A few minutes early
Please arrive about 10 to 15 minutes before your first appointment.
If you have records from a previous doctor that are relevant, bring them or ask that office to send them over.
